12th Grade Immunization Requirements

Dear Parent/Guardian of 12th graders:

Ready or not, your student will soon be a senior. I am sending this as a reminder of immunization requirements for all 12th grade students.

NC law requires every student entering 12th grade to have a booster dose of meningococcal conjugate vaccine .

Please contact your child’s healthcare provider or the Cabarrus Health Alliance (704-920-1205) to schedule an appointment to get these important vaccines.

Please remember to ask for an updated copy of your child’s vaccine record so that it can be provided to the school.
